Which of the following wording of the du’a is correct?
اللهم إني أعوذ بك من البرص، والجنون، والجذام، ومن سيئ الأسقام
or
اللهم إني أعوذ بك من البرص والجنون والجذام وسيئ الأسقام

Both versions are correct.
The du’a is recorded in Sunan Abi Dawud, Musnad Ahmad and Kitabud Du’a of Imam Tabarani with the following words (with من):
اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ البَرَصِ، وَالْجُنُونِ، وَالْجُذَامِ، وَمِنْ سَيِّئِ الْأَسْقَامِ
(Sunan Abi Dawud, Hadith: 1549, Musnad Ahmad, vol. 3 pg. 192 and Kitabud Du’a, Hadith: 1342)
Imams Ibn Hibban and Abu Dawud At Tayalisi have recorded the du’a with the following words (without من):
اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْبَرَصِ، وَالْجُنُونِ، وَالْجُذَامِ، وَسَيِّئِ الْأَسْقَامِ
(Sahih Ibn Hibban; Al Ihsan, Hadith: 1017, Musnad Abi Dawud At Tayalisi, Hadith: 2120)
